UNIVERSITY OF ABUJA STUDENTS ID CARD

*ID CARD: IMPORTANT INFORMATION TO ALL STUDENTS!!!* Following the recent information we got from the Vice Chancellor, we have decided to remind and update you about all that you need to get your School Identity Card. *REQUIREMENTS FOR SCHOOL ID CARD* You are the required to submit the photocopies of the following documents: • UniAbuja Admission Letter • Screening Certificate (Form 01) • Blood Group Result • First School Fee Receipts (100L or 200 for DE candidates) • One Passport Photograph Also, staple the above documents and write the following information behind: • Registration/Matric Number • Full Name • Year of Entry • Department • Faculty • Blood Group • Next of Kin • Phone Number • Signature • NIN *NB:* Medical exercise is *not a MUST* for ID Card enrollment. Nevertheless, it is mandatory for all students to do Medicals and this should be done at the school clinic. Also, Blood Group result can be obtained from any reputable health centre. Please take note. All to be submitted in a school file to: *Location:* School ID card office, ITMS , ICT Centre, Zenith Bank Building, Main campus University of Abuja.
